Moving Out of Our Practice Area

If you move out of our practice area we will ask you to register with a practice closer to your new address.

We will honour any appointments booked within 30 days of the notification letter and will treat any emergency that might arise during this period. However, after that time your medical records will be returned to Primary Care Support England who will forward them to your new doctor.

You will need to register with a new doctor as soon as possible. If you need any information on finding a doctor near to your home you can contact Primary Care Support England on 0333 014 2884 or via their website at www.pcse.england.nhs.uk/contact. The NHS Choices website also provides information on GP services in your area and can be accessed at www.nhs.uk/service-search/.

Once you register with a new surgery, they will arrange for your medical records to be transferred. Should you not have re-registered by this time your registration here will cease and it will no longer be possible to provide you with medical advice. Your medical records will be sent to Primary Care Support England until you register with a new GP.
