If you have already had a Sick Note (Fit Note) for this illness your Doctor may not need to see you to issue an additional Sick Note. Please complete this form. We will contact you to let you know when you can collect your Sick Note or we may contact you to arrange an appointment.

We are not issuing sick notes for those patients with coronavirus. Please note if you need to provide evidence to your employer that you need to stay at home due to coronavirus, you will be able to get this from the NHS 111 Online instead of having to get a fit note from your doctor.
