New Patients How to Register All new patients wishing to register with the practice are asked to complete a new patient registration form and a new patient questionnaire. The new patient questionnaire is helpful as it provides us with information about you in advance of receiving your medical records from your previous doctor. Once completed these forms should be brought to the surgery in person together with a form of photographic identification (such as passport, driving licence), proof of residency within the practice boundary and your National Health Service number (if you have it). If you are registering from overseas please be aware that this does not automatically entitle you to secondary care (for example hospital) under the NHS. Practice Boundary: Our practice area includes Kidlington, Yarnton, Begbroke, Shipton-on-Cherwell, Bunkers Hill, Hampton Poyle, Woodstock, Bladon, Islip village and parts of North Oxford up to the ring road. Temporary Residents You are entitled to see a doctor anywhere in the UK if you are away from home. You can do this by asking to see the nearest doctor as a temporary resident. Similarly we are happy to see friends or relatives who are staying with you, should the need arise. Temporary Residents from Abroad Please note, if you are a foreign national you may not be entitled to NHS treatment and will be required to pay for any treatment you receive. |
