General Information Confidentiality Your medical records are held in strictest confidence. Information is only passed on with your consent, within the confines of the NHS, by law, or if in the public interest. The practice is computerised and certain details of your medical records are kept on computer. These are strictly confidential and the practice is registered with the Data Protection Act. Suggestions and Complaints Your suggestions and ideas are always welcome. If you have a complaint to make, please ask at reception to speak to our practice manager, Sally Mackie. We run an in-house complaints procedure in keeping with national guidelines. Complaints relating to the out of hours service should be directed to the Oxfordshire PCT on 01865 336714. Change of Address or Telephone Number It is important that you inform us of any change of name, address or telephone number. This is so we may keep our records up to date and likewise inform the Thames Valley Primary Care Agency. Your Rights and Responsibilities It is important that you keep your appointments. However, you have the right to cancel your appointment should you wish to. Violent or Abusive Behaviour This practice has a zero tolerance policy for dealing with instances of unacceptable behaviour that cause harm or the fear of harm to any person within the medical centre. This applies throughout the premises, the car parking area and grounds. It also applies to any staff member away from the medical centre in so far as it relates to the business of Gosford Hill Medical Centre. |
